What is the procedure?

If the study is terminated earlier than foreseen in the protocol, inform the MREC of the termination date within 15 days, stating the reason.

What should I submit? 

Send the notification of early termination, along with a cover letter and a B7 form, via email in attachment(s) to info@stbebo.nl.
